Resident Enrollment

Welcome to Liberty Local Schools!  If you are a Liberty resident, please start your registration through FinalForms.  Once you have completed your FinalForms account, please come to your child's building office with the following documents to be copied:

  • Two (2) proofs of residency (one must be a current lease or deed)

  • Child's original birth certificate

  • Child’s Social Security card

  • Print out of immunizations

  • Parent/guardian’s driver's license or state ID

  • Court documents - most recent court order or foster placement paperwork (if applicable)

  • Special education - I.E.P., E.T.R., 504 Plan (if applicable)

  • For 9-12 please bring a copy of the students transcript
