Liberty Local Schools to host vaccine clinic on January 14, 2022

The Liberty Local School District will collaborate with Rite Aid Pharmacy on Friday, January 14, 2022, between 2 p.m. and 5 p.m. to provide a Covid-19 vaccine clinic. This clinic will be provided for teachers, staff, students and any family members. Pfizer vaccines will be provided for all age groups (5-11, 12-15, and 16 and up) Boosters can also be accommodated for the following age groups (16 and up). All children under the age of 16 will need to have a parent present at time of vaccination. There is a required consent form that must be filled out prior to receiving the vaccine. These can be obtained by clicking the attached link below, or they can be picked up in the main office of your child's building. To ensure timeliness, we would encourage you to call to set up an appointment time. Walk ins may be accepted depending on vaccine availability brought in by Rite Aid. To make your appointment, please call Mrs. Amie DeLuca, District RN at 330-759-2301 ext 1110. The vaccine clinic will be held at the Blott Guy Multi-Purpose Room/Cafeteria.
